Do the I still need an Underage Waiver Form after I become a member?Yes, you will need a form each time you come out to play. That is unless you have had it notarized.
Do the Underage Waiver Forms need to be Notarized?No, the forms do not need to be notarized. If the form is signed then it is only good for the day. However if the form is Notarized then it is placed on a underage file until the child is 18, otherwise they will need a new one each time.
